New Access Points
(HRSA - 1 Year)
J2 submitted 9 NAP applications with a total federal funding request of $5.8M. NAP funding supports new health center service delivery sites to expand affordable, accessible, and high-quality primary health care for communities and populations. Funding awards are expected to be announced for FY 2026.
Quality Improvement Fund – Transitions in Care for Justice-Involved Populations
(HRSA - 2 Years)
$1 million awarded to a Texas FQHC to enhance care transitions for individuals nearing release from incarceration, improving their access to community-based, high-quality primary care services.
Service Area Competition (HRSA - 3 Years)
J2 submitted 11 SAC applications with a total federal funding request of $84.5M. SAC funding ensures the continuity of affordable, accessible, and high-quality primary health care in communities and populations currently served by the Health Center Program.
Teaching Health Center Graduate Medical Education (HRSA - 4 Years)
$5.76M awarded to a Federally Qualified Health Center in Louisiana to support the training of residents in a primary care residency program within a community-based health center. This program will prepare residents to provide high-quality care and develop competencies to serve varied patient populations.
Teaching Health Center Graduate Medical Education (HRSA - 4 Years)
$13.2M awarded to two FQHCs in Texas and Georgia to support the training of medical residents in primary care and psychiatry residency programs within community-based health centers. These residency programs prepare residents to provide high-quality care and develop competencies to serve varied patient populations.
Expanded Hours
(HRSA - 2 Years)
$1 million awarded to a FQHC in West Virginia to expand access to health center services by increasing health center operating hours to meet identified patient and community needs.

FY2024
Behavioral Health Services Expansion (HRSA - 2 Years)
$13.2 million awarded to 12 Federally Qualified Health Centers in six states, including Alabama, Florida, Maryland, Pennsylvania, Texas, and West Virginia. This funding will increase access to behavioral health services through starting or expanding mental health and substance use disorder (SUD) services, including Medication for Opioid Use Disorder (MOUD).
Expanding Health Care Access and Promoting Health Equity (MD Community Health Resources Commission
- 3 Years)
$335,000 awarded to a Federally Qualified Health Center in Maryland to increase access to comprehensive, people-centered healthcare to improve chronic disease outcomes among individuals with prediabetes, diabetes, and hypertension.
State and Regional Primary Care Association (PCA) Cooperative Agreements (HRSA - 3 Years)
$6.7 million awarded to two PCAs in Louisiana and Arkansas to develop and deliver training and technical assistance (T/TA) that supports health centers to enhance individual and community health. PCAs assist health centers in expanding access to comprehensive, people-centered, and high-quality primary care services, recruiting and retaining a well-rounded health care workforce, strengthening preparedness and response to emergent health events, implementing value-based care models to ensure financial sustainability, and advancing data-driven improvements in operations, clinical quality, and care coordination.
USDA Distance Learning and Telemedicine - Grant Award
$484,000 awarded to support the expansion of infrastructure for enhanced telehealth services at a Federally Qualified Health Center in West Virginia.
FQHC Incubator Project Based Program (TDSHS - 1 Year)
$1.3 million awarded to two Texas FQHCs to enhance clinical operations and services through the Federally Qualified Health Center Incubator Program.
Rural Health Network Development Planning Grant (HRSA - 1 Year)
$100,000 awarded to an FQHC in Texas to promote the planning and development of a rural integrated health care network. This network will expand access to comprehensive weight-management health and education services, behavioral health resources, and peer support groups for patients who have undergone gastric bypass surgery or are at-risk of obesity.

Advancing Equity in Adolescent Health through Evidence-Based Teen Pregnancy Prevention Programs and Services
(DHHS - 5 Years)
$9.5 million (1.9M per year) awarded to an FQHC in Texas to improve sexual and reproductive health outcomes, promote positive youth development, and advance health equity for adolescents, their families, and communities through the replication of medically accurate and age-appropriate evidence-based teen pregnancy prevention programs (EBPs).
Sentara Cares Health Equity Programs
$500,000 awarded for the Community Engagement, Education, Other Social Determinants Grant Category for an FQHC in Virginia.
Rural Health Care Coordination Program
(HRSA - 4 Years)
$1.2 million ($300K per year) awarded to a non-profit organization in Louisiana to promote rural health care services outreach, and the improvement and expansion of health services in rural areas through care coordination strategies throughout the state.
Congressionally Directed Spending Award - WV
$5 million awarded to a rural FQHC in West Virginia for a new build construction project.
ACGME Accreditation for Teaching Health Center Program
ACGME accreditation awarded to an FQHC in Texas for a new psychiatry residency program, with a letter of recognition for substantial compliance.
Advanced Nursing Education Nurse Practitioner Residency and Fellowship
(HRSA - 4 Year)
$2.8 million ($700K per year) awarded to an FQHC in Louisiana to develop and implement the state's first Nurse Practitioner (NP) Residency program in a community-based setting, allowing an expansion and development of a qualified primary care health workforce, and increase in sorely needed access to preventative services.
Early Childhood Development (HRSA - 2 Year)
$800,000 ($200K per year) awarded to two FQHCs in Pennsylvania and Louisiana to improve developmental outcomes among children ages 0-5, increase developmental screenings for children, increase access to follow-up care, and integrate early childhood development expertise into care teams.
Capital Assistance for Hurricane Response and Recovery Efforts (CARE) (HRSA - 3 Year)
$166,000 awarded to an FQHC in Florida to support capital needs such as alterations, renovations, or equipment costs to meet the needs of areas affected by Hurricanes Fiona and Ian.
Congressionally Directed Spending Award - VA
$800,000 awarded to an FQHC in Virginia to develop a construction project in an underserved community.
USDA Distance Learning and Telemedicine - Grant Award
$416,000 awarded to upgrade telehealth equipment and technology at more than a dozen locations for an FQHC in West Virginia.
Health Center Program School-Based Service Expansion (HRSA - 2 Year)
$2.4 million ($500 - $700K per year) awarded to four FQHCs in Louisiana, Texas, and West Virginia to expand access to primary care and behavioral health services by enhancing services and staff at new and/or existing school-based service delivery sites.
Rural Communities Opioid Response Program – Overdose Response (HRSA - 1 Year)
$300,000 awarded to an FQHC in Alabama to address immediate substance abuse needs in rural communities by expanding access to care and enhancing the capacity for treatment and recovery services for patients living with one or more substance use disorders (SUD).
HRSA Initial FQHC Look-Alike Designation Award
Secured the initial FQHC Look-Alike Designation for a New York Community Mental Health Center, which involved preparing a competitive application and assistance with HRSA's Operational Site Visit.
Appalachian Regional Commission - Inspire Initiative Grant Awards
$50,000 awarded to two health care organizations in West Virginia for a planning and development grant to create a substance use disorder treatment network, partnering with external organizations to pipeline patients in recovery to educational and employment opportunities while offering behavioral health services.
